Your Band / Company Name (or abbreviation);
Short Description of what your image looks like;
and File type (i.e. tiff, jpeg, ai, etc...).

Here's an example of how to name your artwork:
bandname.description.filetype
Ublisch.FlamingSkull.tiff
Ublisch.NuallaLogo.jpeg

PLEASE DO NOT name your file something vague like t-shirt.jpg, t-shirt.psd, scan.jpeg, etc.
Artwork with vague names can create problems trying to match up art to verification requests and can result in a delay in responding to you.

TIP #1! You must name / rename your artwork file to the above naming format before attempting to upload the file after submitting this form.

TIP #2! Do not use any spaces or create file names longer than 32 characters; you will not be able to upload the files.
